before the AFL ablation within the same patients,” said study first author Thomas Dewland, MD, cardiac electrophysiology fellow at UCSF.Īblation also led to an 11 percent reduction in risk for health care for AF but only a negligible drop in risk for acute stroke. “We think our most convincing finding was that health care utilization significantly dropped after vs. Performing a multivariate analysis adjusting for patient demographics and comorbidities, the UCSF researchers determined AFL catheter ablation reduced the risk for overall hospital-based health care by 6 percent, inpatient hospitalization by 12 percent and emergency department visits by 40 percent. Of the 33,004 patients with an AFL diagnosis (in the absence of an AF diagnosis) and who were observed an average of 2.1 years, 2,733 (8.2 percent) received a catheter ablation. Using the California Healthcare Cost and Utilization Project database of community and academic hospitals across California, Marcus and his colleagues identified patients undergoing AFL ablation from 2005-2009. Small studies have been limited to single academic centers or carefully selected randomized trial participants, and results have not been replicated through subsequent studies. The tip of the catheter either delivers heat or extreme cold to destroy tissue responsible for the initiation and/or perpetuation of the abnormal heart rhythm.Īblation has a high rate of success and symptom improvement, but little has been known if the procedure also results in reduced use of hospital services and lower risk for atrial fibrillation or stroke in a large, real-world population. In the minimally invasive procedure, thin, flexible wires called catheters are inserted into a vein and threaded into the heart. “It’s a safe procedure we frequently perform, even on people in their 90s.”ĪFL can be treated through catheter ablation, especially if medications or electrical cardioversion (shocking the heart back to a normal rhythm) are unsuccessful. “The use of catheter ablation for AFL is very effective and can significantly reduce hospital visits and demand on health care services,” Marcus said. In AFL, the electrical activity is more coordinated into one rapid circuit, but the atria contract very rapidly. This activity is chaotic, and the atrial walls quiver rather than contract normally in moving blood to the lower chambers (ventricles). In AF, electrical impulses are triggered from many areas in and around the upper chambers (atria) of the heart instead of just one area. “Based on our study findings, physicians and patients need to be educated that atrial flutter can be readily cured through catheter ablation, and the procedure may reduce the risk for atrial fibrillation.”Ītrial flutter (AFL) is a common abnormal heart rhythm similar to atrial fibrillation (AF). “We’ve seen firsthand in our clinical experience that atrial flutter is difficult to control with drugs, even more than atrial fibrillation,” said senior author Gregory Marcus, MD, director of clinical research in the UCSF Division of Cardiology. The study is in the July issue of PLOS ONE and available online. Use of catheter ablation is not only beneficial for treating atrial flutter but also can significantly reduce hospital visits – both inpatient and emergency – and lower the risk for atrial fibrillation, according to research by UC San Francisco. If you do wish to continue using both your Mac and Microsoft Office, the upgrade options are relatively straightforward. Anytime you avoid updates you’re opening your systems up to the potential for security vulnerabilities such as malware and ransomware. While perhaps the simplest option for Office for Mac aficionados is to delay upgrading their OS to High Sierra, this is a short-term solution at best. While neither set of developers are saying that Office 2011 for Mac and High Sierra are completely incompatible, they are stating that users are on their own and completely unsupported if they choose to go in this direction. Taken together, these two facts mean it could be nearly impossible to run High Sierra with Office 2011 without significant problems - if it works at all. Not only is Microsoft ending support for Office 2011 for Mac, the new macOS High Sierra will not support 32-bit apps like Office 2011 “without compromises”, as their developers say. Making a shift can be a jarring transition, as users get used to keyboard shortcuts and the ability to collaborate with other Office users. With the Microsoft team dropping support for Office 2011, you do have several options for office productivity software such as upgrading to Office 365, moving to a new perpetual license version of Office 2016 or migrating to a new productivity platform completely. While these perpetual licenses are technically yours to use “forever”, there is a functional lifespan for each release. If you’ve purchased a perpetual license version of Microsoft Office for Mac in the past, it may be time for an upgrade. The popularity of cloud-based software and storage solutions makes this decision easier than ever - a clean install clears out any lingering after effects from previous software. Many users prefer to run a clean version of a new operating system and install from scratch, while others simply run the upgrade and hope for the best. If there are files behind your partition, you’ll want to transition those back to your core before removing the partition, too. If you’re already test-driving a beta version of High Sierra in a partition or otherwise, downloading the update is a snap. Create a clean backup of your entire system “just in case”.Check your available space, as you will need several GB’s of free space for the download.Upgrade your apps to ensure compatibility with the new OS.However, there are still a few steps you’ll want to take before you make the jump, including: The majority of Macs that are newer than 2010 are fully compatible with the new OS, making the free upgrade very attractive to users. The recently-released High Sierra OS has been in beta testing for several months now, giving users plenty of time to preview the new functionality. This makes it particularly challenging when Microsoft decides to deprecate support for a particular version, as they are with Office 2011 for Mac for users who update to the new Mac OS 10.13, codenamed High Sierra. While Apple now has the power to encourage users to move in different directions for their office productivity software, the prevalence of Microsoft Office and Excel dictates that many Mac users still require access to a version of the platforms for their OS. While the early years may have been all about how Microsoft could support Apple’s vision, the current reality is quite different. The Mac to MS Office relationship has been rocky at best, which started decades ago with the competitive relationship between Steve Jobs and Bill Gates in the 1970s. It may be no surprise that when you Google “MS Office for Mac”, that the majority of the results returned are alternatives to the popular office productivity software. Even Office 2016 users may encounter difficulties. Mac users of Microsoft Office 2011 may be in for a nasty shock after upgrading to High Sierra, as Microsoft has announced that it’s no longer supported. These fast-growing vines can extend to more than 15 feet long with numerous branches. Once leaves appear, the plant’s vines will sprout tendrils and start climbing. Soon after germination, the plant will grow three to four leaves. The plant can be transplanted or grown from seeds, which require warm and moist conditions to germinate. While typically cultivated in the field, it can also be grown in a greenhouse. The plant does best in lowland areas to altitudes of up to 3,200 feet. In the tropics, the gourd can be planted year-round as long as crops are properly irrigated during drier months. While technically a fruit, bitter melon is eaten as a vegetable and incorporated into dishes to add a delightfully acidic flavor. To a lesser degree, bitter melon is grown as a specialty vegetable in subtropical regions of the United States, including Florida, Hawaii, and California. But I really felt that Metallica was my calling. We’re all close to this day, but there was a lot of guilt there for a while. I’ve known Tom Hunting since I was 16 years old, I’ve known Gary Holt since I was 17. I didn’t feel guilty about that, but I did feel guilty about leaving the band I started in high school. “They came from songs I had written, music I had written,” he told Metal Hammer in 2016. Like the title track, this Ride The Lightning album track featured ideas Hammett brought along from his old band, Exodus – in this case the riff in the verse is based around that band’s then-unreleased song Impaler. Where the debut was a blur of speed and attitude, the follow-up was the work of a band exploding with ideas and confidence. But the musical gulf between the two was immense. Metallica’s second album, Ride The Lightning, was released on J– just a year and two days after Kill ’Em All. “It was very nerve-wracking to be a huge fan of the biggest band in the world and to want to impress them,” admitted director Roboshobo to Metal Hammer, “so I was relieved to have their approval.”Ĥ6) Trapped Under Ice (Ride The Lightning, 1984) Launched with its own hoax backstory – with Kirk Hammett gamely claiming he bought this mysterious film in Russia and became obsessed with it – the band-free promo combines mock-historic documentary footage of Soviet experiments in tissue revival with effective stylised zombie apocalypse animation. This song’s video is no doubt at least partially responsible for its popularity among fans. This back to basics approach yielded dividends – not least on the likes of All Nightmare Long, a creeping eight minute epic with more pummelling killer riffs than you could shake a stick at. And so when the quartet prepared to record its follow-up set, much was made of the fact that this was Metallica ‘returning to the roots’, with the band and producer Rick Rubin citing …And Justice For All and Master Of Puppets as inspirations for the record. Anger album was difficult for James Hetfield and Ulrich to ignore. Most importantly, for all its surface chaos, the song conveyed the simplest of messages: Metallica are back – don’t fuck.Ĥ7) All Nightmare Long (Death Magnetic, 2008)Īs stubborn, uncompromising and pig-headed as Metallica have always been – and those are compliments by the way – the brutal fan reaction to the unloved St. Still, the title track was one of the album’s undeniable high points: a crunching tour de force of snatched riffs and a melee of drum sounds, poised and cleverly layered with Hetfield’s voice double-tracked spoken and sung over a riff reminiscent of Battery. Anger was the first album since Kill ’Em All that truly reflected who the people in Metallica were: conflicted, overstretched, insanely rich, and now, suddenly, immensely self-doubting. Anger is the album that many Metallica fans still love to hate – just check out the myriad memes it spawned in its wake – which explains why its scant entries in this list are firmly consigned to the higher numbers.Īs the Some Kind Of Monster documentary starkly and deftly illustrates, Metallica were at their lowest ebb both in regards to their personal relationships and their art when it came time to make their eighth studio album. In many ways, St. By the time we sat down to write the next batch of songs, we started getting into things like fear and manipulation, the idea of being trapped in situations you can’t get out of.” As we started figuring out what we were doing, we deliberately started to move away from what we perceived to be the heavy metal clichés – the sword and sorcery imagery, the leather and studs, and all that. “‘ Leather and metal are our uniforms’? I wish I could tell you 35 years later that it was a kind of pisstake, but there wasn’t a lot of irony present in Southern California in the early 1980s. “Obviously, a song like Metal Militia has a different lyrical vibe to it than some of the stuff we came up with over the next few years,” Lars Ulrich told Metal Hammer in 2016. Never mind how far the band have since strayed from their original ideals – this is the essence of thrash. Steeped in NWOBHM thunder and powered by hardcore punk snot ‘n’ bile, Metallica were the ultimate sonic kick up the backside for a flagging scene and songs like this one redefined what it meant to be fast, furious and heavy as fuck. While it is eminently credible to attribute the genesis of thrash to bands that were operating prior to Metallica’s arrival on the metal scene – Motörhead, Venom, Anvil and Accept all have a decent claim to have kick-started the whole thing – there is little doubt that the official starting point for the genre is the band’s debut, Kill ‘Em All.
